INSTRUCTIONS FOR COMPLETING THIS FORM:

  1. Each teacher may bring a maximum of 15 students from each school they serve (grades 3 through 6). Special permission is required for exceptions of any type (from Amy Anderson, aanderson@rrps.net).

  2. The teacher bringing students must be an active member of NMMEA/NAfME; Your NAfME member number and expiration date are required on this form.

  3. You must bring at least 1 chaperone for each 5 students attending (in addition to yourself, the music teacher). However, you do not need a chaperone for every group you assign students to.

  4. In order to participate in the festival, this form must be submitted by January 25, 2012.

  5. If you teach at more than one school, please submit separate forms, one for each school.

  6. There is a festival fee of $20 per group plus $1 per student. This includes cost of T-shirts for all students.  For adult T-shirts there is an additional charge of $7 per shirt.  Be sure to indicate shirt sizes on the form below.

  7. NOTE: You may have to move your view (on this webpage) to the right to enter the chaperone shirt information.

  8. Your purchase order or check must be received by the District VI treasurer 2 days before the festival, or you must bring payment with you on the day of the festival along with the invoice form which will be sent with your festival packet. There will be no refunds for students who fail to attend. ALSO, PER NMMEA POLICY, THERE IS A $100 LATE FEE IF YOUR CHECK OR P.O. IS RECEIVED AFTER THE EVENT.

  9. Your festival packet will include student registration forms. Do not submit them in advance. Each teacher will be responsible for having student registration forms with them at the festival so they can be referred to if needed for emergency information.

  10. PLEASE DO NOT SUBMIT THIS FORM UNTIL YOU HAVE COMPLETE INFORMATION including group, grade and T-shirt sizes.  If you have a problem meeting the registration deadline, please contact James Cooper jmc@modeZ.com.

  11. If you have any questions, put them in the "Notes or Comments" section below.

  12. If you make an error or need to make a correction, please submit the corrected information using this form again, and put an explanation in the "Notes or Comments" section.
